Report Requirements

Report 1:

  1. TAT

Report 2:

  1. TAT, CAT, or RAT
  2. Drawings
  3. Sentence completion

Report 3:

  1. MMPI-A RF (14-18), MMPI-2/RF (18+), MACI (13-19), or M-PACI (9-12)
  2. Other self-report (Connors-3, YSR, BYI-2, BASC-3, BDI-II, etc.)
  3. Other informant report (Connors, CBCL, BASC-3, etc.)
  4. Drawings
  5. Sentence completion

Report 4:

  1. TAT, CAT or RAT    
  2. Drawings
  3. Sentence completion
  4. Self-report measure (YSR, BASC-3, BDI-2, MMPI-A/RF, etc.)
  5. Other-informant measure (teacher or parent or caregiver)
  6. Genogram
  7. Diagnostic Interview

Report Due Dates

Report 1: Due date is 9/29

Report 2: Due date is 10/27

Report 3: Due date is 11/24

Report 4: Due date is 12/15

Report Notes

  1. All reports are based on testing of a different child/adolescent, and will include reason for referral, background information, behavioral observations, test results, and summary. Report 4 will also include recommendations.
  2. All protocols and reports are due on the due date at 5 pm EST. Reports submitted after the due date will be subject to loss of points. Late reports will be reduced the following point values when late:
    1. 5 points when submitted after the due date, up to the following class (1 week)
    2. 5 points every week thereafter
    3. Reports not submitted with no advance notice to the professor (communicating when it will be submitted or other prior arrangements) will receive a zero. This includes the final report (#4).
  3. Reports will be typed double-spaced with 12 pt. font and 1” margins. All reports must include
    1. Rubric scoring form
    2. Completed consent form.
    3. Peer review form. Reports may lose points if not adhering to these guidelines.
  4. Reports and protocols must be submitted as ONE file (Word or PDF) or in ONE compressed (zipped) file. The file is emailed to the assigned grader by the due date.
  5. Reports cannot be resubmitted for a higher grade. If you have concerns about your report, see the instructor or TAs to get feedback BEFORE the due date.
